Subject: Dispute of Account Reported Without Demonstrable Permissible Purpose – 15 U.S.C. §1681b
To Whom It May Concern,
I am writing to formally request an investigation into several accounts currently appearing in my Experian consumer report. I do not recall authorizing, opening, or consenting to the reporting of the accounts listed below. I have no knowledge of signing any contract, disclosure, or agreement that would permit these entities to furnish information to my consumer report.
Under the Fair Credit Reporting Act (15 U.S.C. § 1681b), a consumer report may only be furnished or maintained when there is a valid permissible purpose. At this time, I have not been provided with any documentation demonstrating such permissible purpose, including written authorization bearing my signature.
Additionally, pursuant to 15 U.S.C. § 1681i, I am exercising my right as a consumer to dispute the accuracy and legitimacy of the following accounts and request that Experian conduct a reasonable reinvestigation:
According to FCRA Section 605B (15 U.S.C. § 1681c-2), once an identity theft report is submitted (FTC Report #: xxxxxxxxxxxx), credit reporting agencies are required by law to block any disputed information resulting from identity theft within four business days.
The account listed below was not authorized by me and is the result of identity theft:

If these accounts cannot be verified with competent evidence, including documentation showing my authorization, they must be deleted from my consumer report in accordance with the FCRA.
I respectfully request the following upon completion of your investigation:

2. Deletion of any account that cannot be fully verified.
3. An updated copy of my consumer report reflecting any corrections made.
This is my first formal dispute regarding these accounts. I appreciate your attention to this matter and expect your response within the timeframe established by law.
